How do I make sure my mail in voters ballot gets to me after

I recently became a permanent mail voter but moved shortly after becoming one. I didn't know who to contact regarding my change of address. I changed my address with the DMV and had my mail forwarded through the postal service but never got my mail in ballot. What did I do wrong? Anyone know?
